Manejo interdisciplinario para una óptima rehabilitación oral / Interdisciplinary management for optimal oral rehabilitation

Las estrategias para el éxito en la rehabilitación bucal requieren de la interrelación de varias disciplinas que en conjunto logren resultados predecibles y duraderos. La visión individualizada de cada área de especialidad puede conllevar a no ofrecer la mejor alternativa de tratamiento, es por ello que la valoración, el diagnóstico y la planificación del caso clínico debe ser realizada por un equipo interdisciplinario para evitar esta situación y crear una sinergia en donde el «todo sea mayor que la suma de sus partes¼. El objetivo de este trabajo es presentar un caso clínico en el cual intervinieron varias áreas de especialidad: periodoncia, prostodoncia, cirugía oral y patología bucal, logrando devolver la función y la estética a través del manejo interdisciplinario (AU)

The strategies for success in oral rehabilitation require the interrelation of several disciplines, which together, achieve predictable and lasting results. The individualized view of each specialty area may lead to not offering the best treatment alternative, which is why the assessment, diagnosis, and planning of the clinical case must be carried out by an interdisciplinary team to avoid this situation and create a synergy in where the «whole is greater than the sum of its parts¼. The objective of this work is to present a clinical case where several areas of specialty intervened: periodontics, prosthodontics, oral surgery, and oral pathology, thus achieving the return of function and aesthetics through interdisciplinary management (AU)

Importancia estética de la prótesis inmediata / Cosmetic importance of the immediate prosthesis

Se presentan los casos clínicos de 3 adultos con dientes extraídos, atendidos en la consulta de prótesis dental del Policlínico Docente 30 de Noviembre" de Santiago de Cuba, a quienes se les colocó prótesis inmediata como única solución ante el deterioro bucal que presentaban. Con el proceder realizado se logró la recuperación estética de los pacientes, así como la reinserción en las actividades sociales y laborales, evitando así las afectaciones psíquicas que suelen aparecer

The case reports of 3 adults with extracted teeth are presented. They were assisted in the dental prosthesis service of 30 de Noviembre Teaching Polyclinic in Santiago de Cuba to whom immediate prothesis were placed as the only solution due to the oral deterioration that presented. With this procedure the cosmetics of the patients was recovered and their reintegration to the social and labor activities was achieved, with which the psychic disorders that usually appear were avoided

Prótesis inmediata con analgesia acupuntural en una adulta mayor / Immediate prosthesis with acupunctural analgesia in an elderly

Se describe el caso clínico de una paciente de 62 años de edad, quien temía a los anestésicos dentales convencionales, pero tenía gran preocupación por rehabilitar su estética con prótesis inmediata. Se decidió aplicar anestesia acupuntural y realizar las extracciones dentales para que recibiera el tratamiento deseado. No se manifestaron reacciones adversas y la paciente quedó satisfecha por los resultados obtenidos, así como por las ventajas que ofrece esta terapia natural.

The case report of a 62 years patient is described who was afraid of the conventional dental anesthetics, but she had great concern to rehabilitate her aesthetics with immediate prosthesis. It was decided to apply acupunctural anesthetic and to carry out the dental extractions so that she received the desired treatment. There were not adverse effects and the patient was satisfied with the results obtained, as well as with the advantages of this natural therapy.

De las prótesis inmediatas a la sobredentadura: Reporte de un caso / From immediate prostheses to overdentures: Case report

En el presente artículo se describen los procedimientos clínicos y de laboratorio que se emplearon para rehabilitar a un paciente parcialmente edéntulo maxilar y mandibular. El tratamiento fue dividido en dos fases. La primera correspondió a la inserción de una prótesis total maxilar inmediata y a una sobredentadura mandibular inmediata con esquema oclusal balanceado. Una vez estabilizados los tejidos del paciente, se realizó la segunda fase del tratamiento, en la cual se cambiaron las prótesis inmediatas por una dentadura maxilar convencional y una sobredentadura dentomucosoportada retenida por aditamentos axiales radiculares. En esta segunda fase del tratamiento se reprodujo la guía anterior de las prótesis inmediatas y se empleó un esquema oclusal lingualizado. Los procedimientos descritos permiten disminuir tiempos clínicos y de laboratorio sin mermar la calidad de la rehabilitación final.

The present article purports a description of laboratory and clinical procedures used to rehabilitate a partially maxillary and mandibular edentulous patient. Treatment was divided into two phases. The first phase encompassed the insertion of an immediate maxillary total prosthesis as well as an immediate mandibular overdenture with balanced occlusal scheme. Once the patient's tissues were stabilized, the second phase of the treatment was undertaken: immediate prostheses were exchanged for a conventional maxillary denture as well as an tooth-supported overdenture retained by root-axial stud-type devices. In this second phase, the immediate prostheses' anterior guide was reproduced and a lingualized occlusal scheme was used. Described procedures allowed the decrease of clinical and laboratory time without endangering the quality of the final rehabilitation.

Prótesis inmediata post alveolectomía correctora y tuberoplastia bilateral a propósito de un caso clínico / Inmediate denture post alveolectomy and hyperplastic tuberosity reduction case report

Introducción: la prótesis Inmediata es una alternativa de tratamiento protésico que sustituye inmediatamente los dientes extraídos. Generalmente se confecciona para reponer dientes anteriores, evitando el deterioro psíquico del paciente; en ocasiones, se hace necesario realizar además de las exodoncias de los dientes afectados, técnicas de cirugía preprotésica, como la Tuberoplastia o la Alveolectomía. Objetivo: mostrar que es posible realizar el tratamiento de rehabilitación con prótesis inmediata luego de una amplia resección quirúrgica. Presentación del caso: se presentó el caso de una paciente femenina de 58 años de edad, trabajadora activa, con antecedentes de buena salud. Refiriendo rechazo al período de desdentamiento. Al examen intrabucal presentaba incisivos centrales superiores con fractura oblicua mesial y movilidad de grado III por trauma, protrusión dentoalveolar marcada del sector anterosuperior, con resalte aumentado y reducción del espacio interalveolar en el sector posterior. Fue diagnosticada como desdentada parcial superior clase I, subclase I de Kennedy, y desdentada parcial Inferior clase I de Kennedy. Se decidió rehabilitar a la paciente por prótesis inmediata total superior, realizando previa Alveolectomía correctora y Tuberoplastia bilateral en una misma intervención quirúrgica. Resultados: con el tratamiento quirúrgico-aparatológico elegido se lograron excelentes resultados, mostrando las bondades de este tratamiento. Conclusiones: se obtuvo en la paciente buen resultado estético, funcional y psíquico.

Introduction: the Immediate Denture is a prosthetic treatment option. It substitutes the extracted teeth immediately. It is generally made to restore Front teeth, avoiding the patient's psychic trouble, sometimes becomes necessary to practice previous prosthetic surgery technics, such as reduction of hyperplastic tuberosities or Alveolectomy besides to make the exodontics of affected teeth. Objective: to show that it is possible to carry out the rehabilitation treatment with immediate prosthesis after a surgical wide resection. Case presentation: we presented a 58 year-old feminine patient, active worker with antecedents of good health. Besides she refereed she couldn't see herself without teeth. To the clinical exam it presents upper central incisors with mesial oblique fracture and III mobility grade, for trauma. Also shows marked dental and alveolar protrusion of the upper front sector, with increased overjet and overbite. And reduction of the interalveolar space in posterior area. Being diagnosed as upper partial toothless Kennedy's class I, subclass I, and inferior partial toothless Kennedy's class I.We decided to rehabilitate the patient for immediate complete upper denture carrying out previous Alveolectomy proofreader and bilateral reduction of hyperplastic tuberosities at the same surgical intervention. Results: with this prosthetic- surgical treatment excellent results were achieved showing the kindness of this treatment. Conclusions: an esthetic, functional and psychic good result was obtained for the patient.

Rehabilitacion oon protesis inmediata en pacientes geriatricos: (presentacion de un caso) / The immediate prosthesis rehabilitation in geriatric patients

En un esfuerzo por ayudar a estos pacientes y teniendo en cuenta las ventajas quirúrgicas, biológicas, estéticas, psíquicas y fonéticas que la rehabilitación con prótesis inmediata representa, hemos incrementado este tratamiento y de esta forma elevar la calidad de vida del adulto mayor. En este trabajo, presentamos un caso de una paciente de 76 años de edad socialmente activa, a quien se le realizó una prótesis inmediata en la Clínica de Prótesis del Policlínico 19 de abril. Se evolucionó clínicamente a las 24 horas para evaluar la cicatrización, dolor, inflamación y la satisfacción de la paciente después de recibir el tratamiento. Se concluyó que la prótesis inmediata actuó como vendaje, protegiendo el coágulo, favoreciendo su epitelización e impidiendo la aparición de infecciones secundarias, con menos dolor, y limitó la inflamación. La paciente sufrió un mínimo de desintegración de su personalidad física y psíquica; se manifestó muy satisfecha con nuestro trabajo.

In an effort to help geriatric patients and having in consideration the surgical, biological, aesthetical, psychological and phonetic advantages that immediate prosthesis rehabilitation provides, we have increased this treatment and by that, we provide a better life style to the elderly. In this work, we present a case of a female of 76 years , socially active, to whom we implemented the immediate prosthesis at The Prosthesis Clinic of the “19 de Abril” Policlinic . She was clinically followed after 24 hours to evaluate scarring, pain, swelling and patient's satisfaction with the treatment. It was concluded that immediate prosthesis acted as a bandage, protecting the clot, favoring epitelization and avoiding the appearance of secondary infections with less pain, limiting swelling. The patient had a minimum of damage to her self esteem, manifesting her agreement with the results of our work.
